In 1887, Eduard Khristianovich Ikavits detailed the buildings of the provincial zemstvo hospital and its territory, as well as the transformations that took place. Here are some excerpts from this description to create an overall picture.

The Tambov provincial zemstvo hospital was built in 1842 with funds from the Public Charity Office and was originally called the "Charitable Institutions of the Tambov Public Charity Office." It was intended not only for the treatment of the sick but also for the care of impoverished elderly people of both sexes in a poorhouse. Additionally, there was a labor and correction house for convicts of both sexes at these institutions. At the end of 1866, all these establishments were transferred to the Tambov provincial zemstvo.

The hospital is located on an elevation, 62 fathoms from the eastern boundary of the northern part of the city. Fields surround it on the north and east sides, while extensive meadows irrigated by the Tsna River and its branches lie to the south. To the east, the hospital is surrounded by fields and brick factories, near which there are ravines. This situation adversely affects the hospital, as manure and other waste are dumped into the ravines, deteriorating air quality.

On the western side, the hospital borders the city. 80 fathoms away are barracks that are not kept as clean as one would like. Particularly poorly arranged latrines contribute to the pollution of the surrounding air. Until 1842, the hospital was housed in an old one-story stone building, next to which was a small wooden house surrounded by a palisade, serving to accommodate a small number of the insane.

All stone buildings of the Charitable Institutions are constructed on a flat area enclosed by a stone wall. On the southern side, it is replaced by a beautiful cast-iron grille on a stone foundation with stone pillars. The facades of almost all buildings face south, except for the former correction and labor house and the former laundry, which now serve as shelters for insane women and men, respectively. Several buildings are located on this territory: in the center, a large three-story building for male patients, to the north of it, a two-story house for insane men with two gardens. Between these buildings is a large garden for male patients to walk in. In 1871, two wooden summer barracks were built in this garden. Between the southern entrance of the large building and the fence, a small garden for female patients to walk in has been laid out. The western corner of the southern side is occupied by a two-story building, where the senior physician's apartment is located on the upper floor, and on the lower floor, the apartment of the steward and priest. The eastern corner houses a similar building for female patients. Around these buildings, flowerbeds have been arranged.

To the south, the courtyard is enclosed by a fence with a gate, and to the west are stone service buildings designated for stables, a pharmacy icehouse with a laboratory, and baths. South of these buildings are wooden sheds, stables, and icehouses, ending 5 fathoms from the building occupied by the senior physician and steward.

A significant renovation of the buildings, especially the large building, was undertaken in 1858 by the Public Charity Office during the governorship of K.K. Danzas and cost over 40,000 rubles. However, the hospital owes its current state to the care and diligence of the Tambov zemstvo, which spared no expense on the expansion and improvement of the hospital, implementing the latest scientific advancements for the treatment of patients and ensuring their comfort. The zemstvo built a small chapel with a room for autopsies and an icehouse for storing the deceased.

In 1871, two summer barracks for cholera patients were built at a cost of about 1,500 rubles, and in 1877, winter barracks on a stone foundation costing 27,000 rubles. Adapting the former labor and correction house into a shelter for incurable men cost 2,000 rubles, while the radical remodeling of the poorhouse for insane women cost 5,500 rubles.

In 1883, construction began on two houses for 106 patients with acute treatable forms of mental illness. The psychiatric hospital with two separate wards and a house for the psychiatrist and staff was completed in 1885 and accepted its first patients the following year.

Currently, this is the main building of the Tambov Regional Hospital.
